## What berberine does

**Berberine** activates AMPK, the cellular energy sensor switched on by exercise and fasting. Meta-analyses of human trials show meaningful improvements in fasting glucose, HbA1c, and lipids. It is not a normal dietary nutrient, so a standardized supplement is the practical route — and exercise and time-restricted eating engage the same pathway.

- **Evidence grade:** Moderate evidence
- **Dose used in studies:** Human trials typically use berberine HCl 500 mg two to three times daily (about 1,000–1,500 mg/day) for 8–24 weeks, lowering blood glucose and lipids comparably to metformin in some studies.
- **Dose from food:** Not meaningfully obtained from food: berberine comes from medicinal plants (barberry, goldenseal, Oregon grape root) used as extracts; edible barberry berries contain only traces, so matching a 500 mg dose from food is impractical.
- **What the grade does not say:** Meta-analyses of dozens of RCTs show metformin-like reductions in fasting glucose and HbA1c (~0.6–0.9%), but the trial base is small, short and mostly Chinese, with no large rigorous confirmatory trials and no hard-outcome data.
