## What it is

Akkermansia muciniphila is a mucin-degrading, Gram-negative anaerobic bacterium that resides in the intestinal mucus layer and typically represents roughly 1-5% of the human gut microbiota. Its relative abundance is often reduced in people with obesity, type 2 diabetes, hypertension and other metabolic disorders. It is regarded as a 'next-generation' beneficial microbe or postbiotic: a specific outer-membrane protein, Amuc\_1100, signals through Toll-like receptor 2, and notably a pasteurized (non-living) form has retained or even enhanced its metabolic effects in preclinical models.

## What the human evidence shows

In a randomized, double-blind proof-of-concept trial in overweight or obese insulin-resistant adults, three months of daily supplementation with A. muciniphila (particularly the pasteurized form, at 10^10 cells/day) was safe and improved insulin sensitivity (+28.6%), insulinemia (-34%) and total cholesterol (-8.7%) while lowering markers of liver dysfunction and inflammation. Follow-up trials have since reported weight-loss maintenance in people with overweight and obesity and improved muscle strength with a pasteurized strain, but results are mixed: a 2025 trial in type 2 diabetes found benefits only in participants whose baseline gut Akkermansia was low, and a larger 2026 multicenter trial in metabolic syndrome missed its primary insulin-sensitivity endpoint overall, with benefits confined to prediabetic, older (63+) and low-baseline subgroups. Mechanistically the bacterium thickens the gut mucus layer, strengthens the intestinal barrier and modulates host immunity. The human trial base remains small—a handful of mostly short trials, several from the same research lineage—with no long-term safety or hard-outcome data, and responses may depend on a person's baseline Akkermansia levels. This evidence describes the microorganism Akkermansia muciniphila and its mechanisms in general, not this specific commercial product.

## Evidence grade: Emerging evidence

- **Dose used in studies:** The key trial (Depommier 2019) gave 10^10 (10 billion) cells/day of pasteurized A. muciniphila for 3 months, improving insulin sensitivity and cholesterol in overweight adults.
- **Dose from food:** Not obtainable from food: Akkermansia is a native gut microbe, not present in any food or normal probiotic; diet can only feed existing colonies via fiber and polyphenols (chicory inulin, cranberry, grapes).
- **Caveat:** A flagship pilot RCT and 2025-2026 follow-up trials show real but modest benefits on insulin sensitivity, weight and HbA1c, but the trial base is small, a 2026 multicenter trial missed its primary endpoint, and benefits may appear mainly in people with low baseline Akkermansia. No long-term safety or hard-outcome data yet.
