## What it is

Probiotics are live microorganisms that, when administered in adequate amounts, are intended to confer a health benefit on the host. Multi-strain formulations combine several bacterial species—most commonly strains of Lactobacillus and Bifidobacterium—delivered at doses measured in colony-forming units (CFU), with high-potency products supplying tens of billions of CFU per serving. They are thought to act by modulating the composition and metabolic activity of the gut microbiota, reinforcing the intestinal epithelial barrier, producing short-chain fatty acids, and interacting with the host immune system and the gut-brain axis.

## What the human evidence shows

The strongest human evidence is indication-specific: a Cochrane review of 31 trials (8,672 participants) found probiotics cut the risk of Clostridioides difficile-associated diarrhea during antibiotic use by about 60% (1.5% vs 4.0%), and another Cochrane review found they modestly reduced the incidence and duration of acute upper respiratory infections. Beyond that, randomized trials and meta-analyses show measurable but generally modest benefits for glycemic control in type 2 diabetes, intestinal barrier integrity, and symptoms of depression, anxiety and stress, while relief in irritable bowel syndrome is strain-selective and low-certainty. In older adults the most consistent finding is a favorable shift in gut microbiota composition, with more variable immune, cognitive and metabolic effects. Notably, a systematic review matching single strains against mixtures within the same indication found multi-strain products are not inherently superior—effects depend on the exact strains and dose, and commercial blends rarely match the studied formulations. Probiotics are generally safe in healthy people, but live-organism products have rarely caused invasive infection in vulnerable hosts such as critically ill or immunocompromised people and preterm infants. This evidence describes probiotic microorganisms and their mechanisms in general, not this specific commercial multi-strain product.

## Evidence grade: Moderate evidence

- **Dose used in studies:** Human trials commonly use about 1–50 billion CFU/day (often 10^9–10^10), with many multi-strain products at 10–25 billion CFU; some high-potency formulas reach 100+ billion.
- **Dose from food:** Fermented foods supply live cultures: kefir roughly 10–30 billion CFU per cup and yogurt about 1–10 billion per serving, so a daily serving can match lower-dose supplements but with fewer, undefined strains.
- **Caveat:** Evidence is indication-specific: moderate-certainty Cochrane support for preventing antibiotic-associated and C. difficile diarrhea and for reducing respiratory infections, but IBS and general wellness benefits are low-certainty and strain-specific. Multi-strain, high-CFU blends are not proven better than single strains, and commercial formulas rarely match the studied ones. Generally safe in healthy people; rare invasive infections have occurred in vulnerable hosts.
