## What it is

Cocoa flavanols are a class of bioactive polyphenols—chiefly the monomer (-)-epicatechin and its oligomeric procyanidins—found naturally in the beans of Theobroma cacao. Cocoa flavanol extracts concentrate these compounds (a commonly studied dose is about 500 mg flavanols per day, including roughly 80 mg (-)-epicatechin) while minimizing the sugar and fat of chocolate. Mechanistically, flavanols are thought to increase endothelial nitric oxide production, promoting vasodilation, and to exert antioxidant and anti-inflammatory effects on the vasculature.

## Its role in longevity

In the large COSMOS randomized trial of older adults, daily cocoa extract did not significantly reduce the primary composite of total cardiovascular events, though cardiovascular death was 27% lower in secondary analyses and an ancillary analysis found the inflammatory marker hsCRP rose about 8% per year more slowly; the trial also showed no benefit for global cognition and no reduction in incident type 2 diabetes. A meta-analysis of randomized trials found improved flow-mediated vasodilation (about +1.2 percentage points), and meta-analyses of shorter trials find a small (about 2 mmHg) blood-pressure-lowering effect. Benefits appear modest, are strongest for vascular function and blood pressure, and several outcomes (overall cognition, diabetes prevention, chronic use in pregnancy) were null, so flavanols are best viewed as one component of a healthy diet rather than a treatment. This evidence describes cocoa flavanols and cocoa extract and their mechanisms, not this specific commercial product.
