## What it is

Coenzyme Q10 (CoQ10, ubiquinone) is a fat-soluble compound the body synthesizes and also obtains in small amounts from foods such as oily fish, organ meats, and nuts. Concentrated in the inner mitochondrial membrane, it serves as an essential electron-transport-chain cofactor for cellular ATP (energy) production and also functions as a lipid-soluble antioxidant. Ubiquinol is the fully reduced form of CoQ10; the ubiquinone and ubiquinol forms interconvert in the body as the molecule shuttles electrons, and ubiquinol has shown better oral absorption than ubiquinone in several pharmacokinetic studies. Endogenous CoQ10 levels fall with age and are further lowered by statin drugs, which inhibit a biosynthetic pathway shared with cholesterol.

## Its role in longevity

In human trials CoQ10 has been studied mainly as an adjunct in cardiovascular disease: the Q-SYMBIO randomized trial reported roughly halved major adverse cardiovascular events in chronic heart failure, and a 2024 meta-analysis of 33 heart-failure trials found lower all-cause mortality (RR 0.64) and hospitalization (RR 0.50), though a Cochrane review still rates the heart-failure evidence as low certainty. Other meta-analyses suggest modest reductions in systolic blood pressure, improved flow-mediated (endothelial) dilation, and small improvements in lipids, glycemic markers, and fatigue, balanced by a Cochrane review finding no clinically significant blood-pressure effect in primary hypertension and one meta-analysis finding no benefit for statin-associated muscle pain. Ubiquinol-specific trials are fewer but intriguing: high-dose ubiquinol (1,500 mg/day) slowed functional decline in multiple system atrophy in a phase 2 trial, and small trials report improved endothelial function. Much of the mortality data (for example KiSel-10, with benefit persisting at 10 years) comes from CoQ10 combined with selenium, no trial shows CoQ10 extends lifespan in healthy people, and findings are best viewed as promising rather than definitive. This evidence describes the coenzyme Q10 compound and its mechanisms, not this specific commercial ubiquinol product.
