## What it is

Myo-inositol is a naturally occurring sugar-like compound (a cyclitol, the most abundant of nine inositol stereoisomers) that the human body synthesizes and also obtains from foods such as fruits, beans, grains, and nuts. It is a structural component of cell membranes and a precursor of inositol-phosphoglycan second messengers that mediate insulin signal transduction, influencing glucose uptake and ovarian function. In insulin-resistant states such as polycystic ovary syndrome (PCOS), altered inositol metabolism is thought to contribute to impaired insulin signaling.

## Its role in longevity

In randomized trials and meta-analyses, myo-inositol (often combined with D-chiro-inositol) has been associated with improved insulin sensitivity—lower fasting insulin and HOMA-IR—restoration of menstrual regularity and ovulation in PCOS, and a reduced incidence of gestational diabetes (a Cochrane review found roughly half the risk, with low certainty), with a favorable safety profile and fewer gastrointestinal effects than metformin. It has also improved metabolic-syndrome markers such as blood pressure, HOMA-IR and lipids in postmenopausal women. Evidence quality is mixed, however: the systematic review informing the 2023 international PCOS guidelines judged the overall evidence 'limited and inconclusive' and found metformin superior on several outcomes, most trials are short, many gestational-diabetes trials are small and from a single country, and aging or longevity outcomes are essentially unstudied. This evidence describes the compound myo-inositol and its mechanism, not this specific commercial product.
