## What it is

A 24-hour ambulatory blood pressure monitor (ABPM) is a portable, automated device that records blood pressure at regular intervals over a full day and night while a person carries out normal activities and sleeps. By capturing daytime, nighttime, and 24-hour average blood pressure, as well as patterns such as nocturnal dipping and short-term variability, ABPM provides out-of-office readings widely regarded as the reference standard for blood pressure assessment. It can identify white-coat hypertension (elevated only in the clinic) and masked hypertension (elevated only outside the clinic), which single clinic readings miss, and major guidelines, including the US Preventive Services Task Force and the 2025 AHA/ACC hypertension guideline, recommend confirming a hypertension diagnosis with out-of-office measurement before treatment.

## Its role in longevity

Large prospective cohorts and pooled analyses of more than 10,000 people show that ambulatory blood pressure, especially the nighttime and 24-hour average, predicts cardiovascular events and all-cause and cardiovascular mortality more strongly than clinic measurement, and that masked hypertension and high nighttime pressure carry substantial risk while white-coat hypertension carries comparatively lower risk; the more of the day readings stay within guideline target ranges, the lower the observed risk. Blood pressure variability and non-dipping also track with worse outcomes, though dipping status is only modestly reproducible and randomized trials proving that ABPM-guided treatment improves outcomes are still lacking — the benefit comes from better hypertension management, not the device itself. This evidence concerns ambulatory blood pressure and its measurement as a biomarker of cardiovascular risk, not this specific commercial monitor device.
