## What it is

An inspiratory muscle strength trainer is a small handheld device that imposes a high resistance on inhalation, so the user breathes in forcefully against a load for a set number of breaths (typically about 30 breaths per day). This form of respiratory exercise, called inspiratory muscle training (IMT) or high-resistance inspiratory muscle strength training (IMST), strengthens the diaphragm and other inspiratory muscles, an adaptation quantified as an increase in maximal inspiratory pressure. Originally developed to help patients with lung disease, the high-resistance, low-volume protocol has more recently been studied for effects on blood pressure, vascular endothelial function and autonomic regulation. Threshold and tapered-flow resistive trainers are the devices most commonly used in this research.

## Its role in longevity

In randomized trials, most prominently sham-controlled work from the University of Colorado in midlife and older adults, about six weeks of high-resistance IMST performed a few minutes a day lowered systolic blood pressure by roughly 9 mmHg and improved vascular endothelial function, with much of the effect persisting six weeks after stopping. Meta-analyses in hypertension report systolic reductions of about 8 to 13 mmHg, and one 2023 meta-analysis using trial sequential analysis judged the blood-pressure evidence conclusive; a trial in older adults with obstructive sleep apnea also found reduced muscle sympathetic nerve activity, a plausible mechanism. Separate trials and reviews link inspiratory muscle training to better cerebrovascular reactivity, respiratory muscle strength, exercise capacity and quality of life across COPD, heart failure and athletic populations. The evidence is tempered by small samples, short follow-up, concentration in a few research groups, and no testing of hard outcomes such as stroke; at least one controlled trial in healthy older adults found gains in inspiratory strength without improvements in systemic inflammation, exercise performance or quality of life. This evidence describes inspiratory muscle training and the underlying breath-loading mechanism in general, not any specific commercial trainer.

But trials are small (about 25-100 participants), short (6-12 weeks), and many come from one research group; hard outcomes like stroke and heart attack are untested. An adjunct to, not a replacement for, exercise and prescribed antihypertensives.
