## What it is

Transcutaneous electrical nerve stimulation (TENS) is a non-invasive, portable method of pain relief in which a battery-powered device delivers low-voltage electrical currents to the skin through surface electrodes. First introduced in its modern form in the 1970s, it is widely used for acute and chronic musculoskeletal and neuropathic pain. Its main proposed mechanism is the gate-control theory of pain: stimulation of large-diameter A-beta sensory fibres inhibits transmission of pain signals carried by smaller A-delta and C fibres at the spinal dorsal horn. TENS is also thought to engage the endogenous opioid system and descending inhibitory pathways - low-frequency stimulation acting through mu- and delta-opioid receptors, with repeated use capable of inducing opioid tolerance.

## Its role in longevity

Evidence on TENS is mixed but, on balance, supportive for short-term pain relief. The largest synthesis (381 randomized trials) found pain intensity lower during or immediately after TENS versus placebo, and randomized trials and meta-analyses report benefits for fibromyalgia, post-surgical pain (with reduced opioid use), and some knee-osteoarthritis outcomes; a 2023 clinical practice guideline suggests TENS as a non-drug option for temporomandibular disorder pain. However, several rigorous studies are null - including a placebo-controlled knee osteoarthritis trial and a chronic low-back-pain meta-analysis - and effects are generally modest and transient, with certainty of evidence often low. TENS is notably safe, with only minor adverse events, but it relieves symptoms rather than modifying any underlying disease. This evidence describes the TENS modality and its underlying gate-control and neuromodulation mechanisms, not this specific commercial product.
