## What it is

An electrical muscle stimulation (EMS) unit, also called a neuromuscular electrical stimulation (NMES) device, delivers electrical impulses through skin electrodes to evoke involuntary skeletal-muscle contractions. By activating motor units directly, these contractions can reproduce aspects of voluntary resistance exercise and are used clinically to preserve or rebuild muscle when normal training is limited by surgery, injury, immobilization, or illness. Whole-body EMS (WB-EMS) systems stimulate several large muscle groups simultaneously during light movement, and NMES preferentially recruits fast-twitch (type II) fibers.

## Its role in longevity

In controlled trials, NMES attenuates muscle loss and weakness during disuse (leg immobilization, ICU admission, and after knee surgery) and increases quadriceps strength, with cellular studies showing preserved muscle-fiber size and contractility. In older adults, whole-body EMS trials and meta-analyses report gains in muscle mass, strength and sarcopenia scores, positioning it as a joint-sparing option for people unable or unwilling to perform conventional resistance training. However, effects are generally smaller than those of voluntary exercise, evidence in healthy older adults remains limited, effects on fat loss and pain are minor or absent, and regulators warn that effortless ab-stimulator marketing claims are unsupported. It is contraindicated with pacemakers and implanted defibrillators and over the anterior neck, pregnant abdomen or broken skin. This evidence describes NMES/EMS as a modality and its underlying muscle-contraction mechanism, not this specific commercial product.

## Evidence grade: Moderate evidence

- **What the grade means:** Curated grade based on the human and mechanistic evidence for this compound.
- **What this grade does not say:** Trials and meta-analyses show NMES or whole-body EMS can preserve or build muscle and strength in older adults and rehab patients, mainly as an adjunct to, not a replacement for, voluntary exercise. Effects on fat loss and pain are small or absent, evidence in healthy users is limited, and effortless-abs marketing claims are unsupported. Avoid with pacemakers or implanted defibrillators.

## In the news

  > the FDA states that most EMS devices they review are intended for use in physical therapy and rehab environments, not for aiding in fat loss.
