## What it is

Azelaic acid is a naturally occurring saturated dicarboxylic acid found in grains such as wheat, rye, and barley and produced on the skin by the yeast Malassezia. Applied topically in cream, gel, or foam formulations (typically 15-20% at prescription strengths, around 10% in over-the-counter cosmetics), it exerts anti-inflammatory, antioxidant, and antimicrobial actions and inhibits the enzyme tyrosinase. It works in part by scavenging free radicals, reducing the reactive oxygen species released by neutrophils, and normalizing the aberrant cathelicidin/kallikrein-5 innate-immune signaling implicated in rosacea, and it is FDA-approved for the topical treatment of rosacea.

## Its role in longevity

Multiple randomized controlled trials, Cochrane reviews, and meta-analyses find that topical azelaic acid significantly reduces inflammatory lesions and erythema in papulopustular rosacea - a systematic review of five trials (873 patients) found four of five superior to vehicle - and a 2019 GRADE-assessed review classed it among the topicals with high-certainty evidence. Head-to-head data suggest it is at least as effective as topical metronidazole, although some analyses rank topical ivermectin as more effective, and it has not been shown to reduce telangiectasia; evidence for acne and melasma is also supportive. One honest caveat: most rigorous trials used prescription 15-20% strengths, so benefit at over-the-counter ~10% cosmetic doses is plausible but less directly proven. Benefits typically take one to two months to appear, and mild, transient stinging or irritation is the most common side effect. This evidence describes the azelaic acid compound and its mechanisms, not this specific commercial gel product.
