## What it is

Topical tranexamic acid (TXA; trans-4-aminomethylcyclohexanecarboxylic acid) is a synthetic lysine derivative originally developed as an oral antifibrinolytic (plasmin inhibitor) to reduce bleeding, later found to lighten ultraviolet- and hormone-driven facial hyperpigmentation such as melasma. Applied to the skin it interferes with the plasminogen-plasmin pathway in keratinocytes, dampening UV-induced prostaglandin and arachidonic-acid signaling to melanocytes and thereby reducing melanocyte tyrosinase activity and melanin production without decreasing the number of melanocytes. It is formulated as a serum or cream, typically at concentrations of 2-5%, for melasma and other facial dyschromia.

In randomized controlled trials and multiple meta-analyses, topical tranexamic acid significantly lowers melasma severity scores (MASI/mMASI and melanin index) and performs comparably to the long-standing standard agent hydroquinone, frequently with fewer irritant side effects; combining it with microneedling or laser-assisted delivery can add further benefit. Honest limitations: many trials are small, short (typically 8-12 weeks) and heterogeneous, melasma tends to relapse after treatment stops, and pooled analyses indicate oral TXA is more potent than the topical form, though the oral route carries clotting-risk contraindications that minimally absorbed topical use largely avoids.
