## What it is

The Withings Body Scan is a consumer smart bathroom scale that estimates body composition using bioelectrical impedance analysis (BIA), a technique that passes a small, imperceptible electrical current through the body and infers fat mass, fat-free (muscle) mass, and total body water from the tissue's resistance. Alongside weight and body-fat percentage, such devices report derived metrics including visceral fat, segmental muscle mass, and—via pulse-wave and electrical sensors on the handle and platform—estimates of vascular age and heart rhythm. The accuracy of BIA depends heavily on the prediction equations and hydration assumptions used, so consumer readings are best interpreted as trends rather than exact laboratory values.

## Its role in longevity

The biomarkers a body-composition scale tracks are each linked to healthy aging: higher body-fat percentage and visceral adiposity are associated with greater cardiovascular and metabolic risk and, in general adult populations, higher all-cause mortality, while low muscle mass (sarcopenia) predicts disability and death in older adults, and arterial-stiffness measures such as pulse-wave velocity forecast cardiovascular events. The behavioral evidence is equally context-dependent: daily self-weighing produced substantial weight loss (−6.6% vs −0.4% body weight at 6 months) in one randomized trial when paired with feedback, but meta-analysis finds self-weighing effective inside structured programs while the one trial of stand-alone weighing advice showed no effect—and frequent weighing should be avoided by people at risk of eating disorders. Consumer BIA fat estimates are group-level approximations (validated equations explain most, but not all, of DXA-measured variance) best read as trends, and several biomarker associations weaken or reverse in the very old. One device-specific validation exists: the Body Scan's sweat-gland (sudomotor) nerve-fiber screening agreed with a hospital reference with about 91% sensitivity and 97–99% specificity. The remaining evidence describes the underlying measurements, not this retailer's listing; educational, not medical advice.

## Evidence grade: Measurement tool

- **What the grade means:** Graded as an instrument: how accurately it measures, and whether measuring it changes anything.
- **Dose used in studies:** Weigh at the same time each morning, after voiding and before eating, and read the trend line over weeks rather than any single day. Use the weight; treat the body-fat, segmental and vascular readings as rough estimates that drift with hydration.
- **What this grade does not say:** The FDA clearance on this scale is narrow and specific: it covers detection of atrial fibrillation from the 6-lead ECG, and nothing else. The bioimpedance body composition is the part most people buy it for, and it is the part that does not hold up at an individual level: against DXA, bioimpedance underestimates body-fat percentage by roughly 5 to 6 percentage points and overestimates fat-free mass by around 4 kg, with limits of agreement wide enough that published comparisons conclude the two methods are not interchangeable for one person, only for group averages. I could not find any published validation of the vascular-age or nerve-health scores against a reference standard. Self-weighing does genuinely help, adding about 1.7 kg of weight loss when built into a behavioural programme, but that benefit comes from stepping on a scale regularly and works on a $25 one; no trial shows that watching your body-fat percentage or vascular age changes any outcome.
