### Spermidine (Wheat-Germ Extract)

- **Evidence grade:** Moderate evidence
- **Dose used in studies:** Cognition trials in older adults used modest, food-level doses of wheat-germ extract: 0.9 mg/day for 12 months (SmartAge) and 1.2 mg/day for 3 months. Higher doses are less studied.
- **Dose from food:** Readily obtained from food: wheat germ holds ~24–35 mg per 100 g (about 2–2.5 mg per tablespoon); natto, aged cheese, mushrooms and soy are rich too. A normal diet supplies ~10–15 mg/day, exceeding the doses tested.
- **What the grade does not say:** The preclinical case is unusually strong and several large prospective cohorts link higher dietary spermidine intake to lower mortality — but that is observational and confounded by diet quality. The most rigorous human trial to date (12-month SmartAge) found no cognitive or biomarker benefit at the 0.9 mg/day dose used in supplements, and oral bioavailability is unsettled.
