Twelve studies involving 357 participants were included. Beetroot juice significantly improved exercise capacity (standardized mean difference [SMD], 0.31). Stratified analysis showed greater benefits in parallel RCTs (SMD, 0.52) compared to crossover trials (SMD, 0.06). BJ also reduced systolic blood pressure by 5.33 mmHg and diastolic blood pressure by 3.01 mmHg.

## Conclusion

This meta-analysis provides strong evidence that beetroot juice improves exercise capacity and key cardiovascular parameters in COPD patients. Stratifying results by study design addressed previous methodological limitations, establishing a reliable foundation for clinical application.
