## Methods

This was a triple-blind randomized clinical trial involving 58 infertile women with PCOS. Participants were randomly assigned to receive either a placebo or ASX (2 × 6 mg/day) for 8 weeks. Blood serum samples were collected before and after the intervention to evaluate fasting insulin (FI), fasting blood glucose (FBS), oxidative stress markers (malondialdehyde [MDA], superoxide dismutase [SOD], and total antioxidant capacity [TAC]), and lipid profiles. Insulin resistance indices were calculated, and blood pressure was assessed at the start and end of the study.

## Results

After 8 weeks, ASX consumption led to significant reductions in fasting blood sugar, HOMA-IR, FI, MDA, low-density lipoprotein-cholesterol, and TC/HDL-C. Conversely, ASX significantly increased TAC, HDL-C, and QUICKI. After adjusting for baseline values of age, BMI, and biochemical parameters, QUICKI and FI showed non-significant values, while other findings remained unchanged.

## Conclusion

Astaxanthin appears to be an effective and safe supplement that improves insulin metabolism, lipid profile parameters, and oxidative stress in infertile PCOS patients.
