## Takeaway

Higher long-term systolic blood pressure variability is associated with increased risks of death and cardiovascular events, particularly stroke.

## The question

Is long-term systolic blood pressure variability (SBPV) associated with cardiovascular (CV) outcomes, even after adjusting for mean blood pressure?

## Hypothesis

The study aims to evaluate the associations between mean blood pressure adjusted long-term SBPV and CV outcomes, suggesting a potential association.

## Methods

A systematic review and meta-analysis was conducted by searching PubMed, Scopus, and Science Direct on January 4, 2023. The analysis included 43 studies with a total of 9,944,254 subjects.

## Results

Long-term SBPV was associated with an increased risk of all-cause mortality (HR 1.21), CV mortality (HR 1.10), major adverse cardiovascular events (MACE) (HR 1.10), cerebrovascular stroke (HR 1.22), and myocardial infarction (HR 1.13). European populations showed a generally higher risk compared to other continents.

## Conclusion

Long-term SBPV is associated with all-cause mortality, CV mortality, MACE, myocardial infarction, and stroke. Poor outcomes linked to long-term SBPV appear to be more driven by cerebrovascular events than coronary events.

## Limitations

The abstract does not explicitly state limitations of the meta-analysis itself, but notes the questionability of the association after adjusting for mean BP, which the study aimed to address.

## Verbatim from the abstract

> long-term systolic blood pressure variability is associated with all-cause and cardiovascular mortality

## Increased Risk Associated with Long-Term SBPV

| Measure | Hazard Ratio |
| --- | --- |
| All-cause mortality | 1.21 |
| Cerebrovascular stroke | 1.22 |
| Myocardial infarction | 1.13 |
| CV mortality | 1.1 |
| MACE | 1.1 |

*Hazard ratios indicate the increased risk of various outcomes associated with long-term systolic blood pressure variability.*
