## Methods

A prospective analysis was conducted on 207,368 UK Biobank participants without prior atherosclerotic disease, diabetes, or active lipid-lowering therapy. Multivariable-adjusted Cox regression models were used to assess the association between incident CAD and nuclear magnetic resonance-measured apoB-P, concentrations of individual lipoprotein classes (VLDL, LDL), size subclasses, average particle diameter, and immunoassay-measured lipoprotein(a) [Lp(a)].

## Results

A one standard deviation (SD) increase in apoB-P was associated with a 33% higher CAD risk (HR: 1.33). VLDL particles showed a higher per-particle risk (HR per 100 nmol/L: 1.22) compared to LDL (HR per 100 nmol/L: 1.07), but this difference was balanced when considering relative particle abundance (LDL 91% vs VLDL 9% of total apoB-P), resulting in respective HR per 1-SD of 1.09 for LDL and 1.24 for VLDL. Particle diameter or size subclasses were not associated with CAD after apoB-P adjustment. Lp(a) showed a robust association even after apoB-P adjustment (HR: 1.18) and provided independent prognostic value for CAD (AUC: 0.769 vs 0.774, P < .001).

## Conclusion

The total count of apoB-containing lipoprotein particles most accurately reflects lipid-related atherosclerotic risk, and this risk is largely unaffected by the major particle type (VLDL, LDL) or size. An elevated count of Lp(a) adds additional risk, suggesting that assessing atherogenic risk from dyslipidemia is best done by considering both apoB-P and Lp(a) concentrations.
