## The question

Does silymarin improve outcomes in patients with nonalcoholic fatty liver disease (NAFLD)?

## Hypothesis

The study implicitly hypothesizes that silymarin, a hepatoprotective extract, may be effective in slowing NAFLD progression.

## Methods

This meta-analysis included 26 randomized controlled trials with 2,375 patients. Data were extracted from PubMed, Embase, Cochrane Library, Web of Science, clinicaltrials.gov, and China National Knowledge Infrastructure. Continuous values were pooled using standard mean difference (SMD) and dichotomous values using odds ratio (OR). Heterogeneity was assessed with Cochran's Q test (Istatistic), and statistical significance was set at P<0.05.

## Results

Silymarin significantly reduced total cholesterol (TC), triglycerides (TG), LDL-C, FI, HOMA-IR, ALT, AST, fatty liver index, and fatty liver score. It also increased HDL-C and significantly improved hepatic steatosis based on liver histology. Specifically, ALT decreased by an SMD of -12.39, AST by -10.97, and hepatic steatosis showed an OR of 3.25 for improvement.

## Conclusion

Silymarin can regulate energy metabolism, attenuate liver damage, and improve liver histology in patients with NAFLD.

## Limitations

The abstract states that the effects of silymarin will need to be confirmed by further research.
