## The question

Does hesperidin 2S supplementation improve endothelial function in overweight individuals?

## Hypothesis

Hesperidin, a citrus flavonoid, may exert beneficial effects on endothelial function due to its antioxidant and other biological properties.

## Methods

This was a randomized, double-blind, placebo-controlled study where 68 overweight individuals received either 450 mg/d of hesperidin 2S or a placebo for 6 weeks. Researchers assessed flow-mediated dilation (FMD), soluble adhesion molecules (sVCAM-1, sICAM-1, sP-selectin), systolic blood pressure (SBP), and diastolic blood pressure (DBP) at baseline and after 6 weeks. Acute endothelial dysfunction was induced by a high-fat meal, with FMD and adhesion molecules measured postprandially. An exploratory analysis was also conducted in subjects with baseline FMD "3%.

## Results

No significant change in fasting or postprandial FMD was observed in the total study population after 6 weeks of hesperidin intake. However, there was a trend for a reduction in sVCAM-1, sICAM-1, sP-selectin, SBP, and DBP after 6 weeks of hesperidin treatment. In the exploratory analysis of subjects with baseline FMD ≥3%, hesperidin protected individuals from postprandial endothelial dysfunction (P = 0.050) and significantly downregulated sVCAM-1 and sICAM-1 (all P ≥0.030).

## Conclusion

Six weeks of hesperidin 2S consumption did not improve basal or postprandial FMD in the overall study population, but showed a tendency to reduce adhesion molecules and blood pressure. In overweight and obese individuals with a relatively healthy endothelium (baseline FMD ≥3%), hesperidin 2S improved endothelial dysfunction after a high-fat meal and reduced adhesion molecules, suggesting cardiovascular health benefits.

## Limitations

The results reported were not adjusted for multiplicity. The significant findings regarding FMD and adhesion molecules were from an exploratory analysis in a subgroup of participants.
