## The question

Is Acetyl-l-carnitine (ALC) clinically effective in treating people with dementia?

## Hypothesis

The efficacy of ALC in cognitive decline remains unclear, with early studies suggesting benefit and later, larger studies not supporting these findings.

## Methods

This review identified 11 double-blind, randomized trials comparing ALC with placebo in people with Alzheimer's disease. Data were extracted and analyzed using Revman 4.1 software, primarily focusing on completers. Outcomes assessed included cognitive effects, severity of dementia, functional ability, and clinical global impression.

## Results

Statistically significant treatment effects favoring ALC were observed at 12 and 24 weeks for the number of patients showing improvement as determined by Clinical Global Impression (OR 2.33 at 12 weeks, OR 3.91 at 24 weeks). However, no benefit was found at 52 weeks for Clinical Global Impression, nor for cognition, severity of dementia, or functional ability when measured continuously. There were no statistically significant differences in adverse events between ALC and placebo groups.

## Conclusion

There is some evidence for ALC's benefit on clinical global impression, but no evidence from objective assessments in other outcome areas. The statistically significant results might be due to chance given the number of comparisons. Currently, there is no evidence to recommend ALC's routine use in clinical practice for dementia.

## Limitations

All included trials focused only on Alzheimer's disease, not other types of dementia. Methodological differences and assessment tools varied widely across early and later studies, making comparisons difficult. The statistically significant findings for clinical global impression might be due to chance given the multiple comparisons. The review primarily used completers' data rather than intention-to-treat.
