## Methods

A Task Force on the prognostic significance of ambulatory blood pressure monitoring wrote a review in preparation for the Eighth International Consensus Conference. This synopsis was amended based on opinions aired at the conference and common ground reached in discussions.

## Results

Ambulatory monitoring refines cardiovascular event prediction beyond conventional blood pressure. White-coat hypertension (clinic BP ">= 140/90 mmHg, daytime ambulatory BP < 135/85 mmHg) carries a lower cardiovascular risk than sustained hypertension. A decreased nocturnal blood pressure fall (<10% of daytime level) is associated with worse prognosis. Some studies show an independent relationship between short-term blood pressure variability and cardiovascular outcome. Ambulatory blood pressure monitoring is a better predictor of maternal and fetal complications in gestational hypertension than office measurement. Ambulatory pulse pressure is a strong predictor of cardiovascular outcome, and the QKD interval correlates with left ventricular mass. Self-measurement of blood pressure is effective in identifying the white-coat effect under standardized conditions.

## Conclusion

Ambulatory blood pressure measurement provides refined prognostic information compared to conventional readings. Further research is needed on the long-term nature of white-coat hypertension, especially in children, and the prognostic accuracy, applicability, and cost-effectiveness of self-measured blood pressure.

## Limitations

Intermittent ambulatory blood pressure monitoring techniques have limitations in quantifying short-term blood pressure variability. Reference values for ambulatory blood pressure in children are currently based on statistical parameters, and functional definitions of ambulatory hypertension are yet to be developed for this group. The issue of whether white-coat hypertension predisposes to sustained hypertension needs further research. More studies are required to fully elucidate the prognostic accuracy of self-measured blood pressure.
