## The question

What is the effect of weighted chain blankets on insomnia and sleep-related daytime symptoms in patients with major depressive disorder, bipolar disorder, generalized anxiety disorder, and attention deficit hyperactivity disorder?

## Hypothesis

The study hypothesized that weighted chain blankets would improve insomnia and sleep-related daytime symptoms in the target patient population.

## Methods

One hundred twenty patients were randomized (1:1) to either a weighted metal chain blanket or a light plastic chain blanket for 4 weeks. The primary outcome was the Insomnia Severity Index, with secondary outcomes including day and night diaries, Fatigue Symptom Inventory, Hospital Anxiety and Depression Scale, and wrist actigraphy for sleep and daytime activity levels. A 12-month open follow-up phase was also conducted.

## Results

At 4 weeks, the weighted blanket group showed a significant advantage in Insomnia Severity Index ratings compared to the light blanket group (P < .001) with a large effect size (Cohen's d 1.90). The weighted blanket also led to better sleep maintenance, higher daytime activity, and reduced daytime fatigue, depression, and anxiety. No serious adverse events were reported. During the 12-month follow-up, the positive effects on sleep were maintained by continuous users, and those who switched from a light to a weighted blanket experienced similar improvements.

## Conclusion

Weighted chain blankets are an effective and safe intervention for insomnia in patients with major depressive disorder, bipolar disorder, generalized anxiety disorder, or attention deficit hyperactivity disorder, also improving daytime symptoms and activity levels.
