## Why that evidence rating

Dozens of device-measured cohorts and several independent meta-analyses agree on a consistent non-linear dose-response, but every study is observational and none randomised anyone to a step count.

## What to look for

- Track a weekly average total, not a daily target; that is the exposure the cohorts measured
- Under about 4,000 steps a day sits below every reported inflection point; benefit appeared above about 3,143 steps in one meta-analysis and 3,867 in another
- Expect the bend between about 5,000 and 7,000 steps, with slower further improvement above it
- Ignore cadence scores for now; three cohorts found no independent effect of intensity after adjusting for total steps

The 10,000-step target is not a finding. It is a product name. As the American Heart Association recounted in 2021, it comes from 1960s Japanese pedometer marketing: the device was sold as the manpo-kei, the "10,000 step meter." One epidemiologist who has since studied the question put it flatly.

> There was not a lot of science behind it.

What there is a great deal of science behind is the shape of the curve. Device-measured cohorts have replaced the self-report questionnaires that used to dominate this literature, and plotting accelerometer-recorded daily steps against mortality gives something more interesting than a threshold: a steep early descent, a bend, and a long slow flattening. Where exactly it bends is a live argument. That it bends well below 10,000 is not.

## Where the curve actually bends

The most comprehensive synthesis is a 2025 systematic review and dose-response meta-analysis in the Lancet Public Health covering 57 studies from 35 cohorts. Inverse non-linear curves for all-cause mortality, cardiovascular disease incidence, dementia and falls all had inflection points at roughly 5,000 to 7,000 steps a day. Against a reference of 2,000 steps, 7,000 steps was associated with 47 percent lower all-cause mortality (hazard ratio 0.53, 95% CI 0.46 to 0.60), 25 percent lower cardiovascular incidence, 38 percent lower dementia risk and 28 percent lower falls risk.

An earlier 2022 meta-analysis in the Lancet Public Health, harmonising 15 device-measured cohorts with 47,471 adults and 3,013 deaths, found hazard ratios against the lowest quartile (median 3,553 steps a day) of 0.60, 0.55 and 0.47, with splines flattening at different points by age: about 6,000 to 8,000 steps a day in adults 60 and older, about 8,000 to 10,000 in those under 60.

A 2023 meta-analysis in the European Journal of Preventive Cardiology, across 17 cohorts and 226,889 participants over a median 7.1 years, expressed it as a gradient rather than a target: each 1,000-step increment associated with 15 percent lower all-cause mortality (hazard ratio 0.85, 0.81 to 0.91), each 500-step increment with 7 percent lower cardiovascular mortality (0.93, 0.91 to 0.95), non-linear, with benefit emerging above about 3,867 steps a day for all-cause and about 2,337 for cardiovascular mortality.

## The bottom of the curve is where the money is

The step counts associated with the largest incremental gains are embarrassingly low. A 2019 study in JAMA Internal Medicine followed 16,741 US women with a mean age of 72 and 504 deaths over a mean 4.3 years. About 4,400 steps a day was already associated with significantly lower mortality than about 2,700, quartile hazard ratios against the lowest ran 0.59, 0.54 and 0.42, and risk declined until roughly 7,500 steps a day before levelling off.

Research presented at an American Heart Association meeting in March 2023 pushed further into older ages: in adults 70 and over, average age 78, each additional 500 steps a day was incrementally associated with 14 percent lower cardiovascular risk, and those averaging about 4,500 steps had a 77 percent lower observed risk than those under 2,000.

A 2024 umbrella review in Preventive Medicine, covering 11 prior meta-analyses plus an updated analysis of 14 cohorts, put the protective threshold at about 3,143 steps a day and the pooled gradient at 0.91 per 1,000-step increment. It also explains much of the noise in headlines: effect estimates varied considerably with how step exposure was categorised. The curve is real; the round numbers quoted off it are artefacts of where authors drew their bins.

## Does walking faster matter? The literature disagrees with itself

Three of the strongest cohorts found nothing for intensity once total volume was accounted for. In the 2019 JAMA Internal Medicine study of older women, stepping intensity was not clearly related to mortality after adjusting for total steps. A 2020 study in JAMA of 4,840 US adults aged 40 and older, with 1,165 deaths over a mean 10.1 years, found peak-30-minute cadence not significantly associated with mortality after adjustment (highest versus lowest quartile 0.90, 0.65 to 1.27). The CARDIA analysis in JAMA Network Open in 2021, following 2,110 middle-aged adults a mean 10.8 years, likewise found nothing for intensity.

Two large UK Biobank analyses found the opposite. A 2022 study in JAMA Internal Medicine of 78,500 adults aged 40 to 79 with wrist accelerometry found higher peak-30-minute cadence associated with additional risk reduction beyond total step volume, particularly for incident disease. A 2022 study in JAMA Neurology of 78,430 UK adults, with 866 incident dementia cases over a median 6.9 years, found a peak 30-minute cadence of 112 steps a minute carried the strongest association in the analysis (hazard ratio 0.38, 0.24 to 0.60).

A genuine, unresolved disagreement between well-conducted studies, worth reporting as one rather than settling by picking a favourite.

## What the evidence does not show

It does not show that walking more makes you live longer. Not one study here randomised anybody to a step count. Every hazard ratio above compares people who already walked different amounts, and the standing objection, that illness reduces walking well before it kills you so the arrow may point backwards, is not something a cohort can settle however many participants it enrols. An open question, and a large one.

It also does not show the same relationship in everyone. A 2023 harmonised meta-analysis in Circulation, pooling eight prospective studies of 20,152 device-measured adults with 1,523 cardiovascular events, found progressively lower risk across step quartiles in adults 60 and older (hazard ratios 0.80, 0.62 and 0.51) but no significant association in adults under 60 (0.79, 0.90 and 0.95). The age-dependence is real, and it complicates any single number.

And it does not show that the curve stops mattering above 7,000. The same literature that puts inflection points at 5,000 to 7,000 also contains that 2020 study in JAMA, where 8,000 steps carried 51 percent lower mortality than 4,000 (hazard ratio 0.49) while 12,000 carried 65 percent lower (0.35), both against the same reference; a dementia analysis whose maximum benefit sat at 9,826 steps a day, with half already reached at 3,826; and a CARDIA cohort where the 7,000 to 9,999 band (hazard ratio 0.28) was not bettered by 10,000 or more (0.45). Flattening is not stopping, and the top of the curve is genuinely unsettled.

## What to actually do, and what number to look at

The exposure carrying all this outcome data is the crudest number your tracker produces: total daily steps, averaged. Not zones, not scores, not a readiness index. Any device that reports a dependable weekly average is instrumented for the job.

- Track a weekly average, not a daily target. The cohorts measured habitual daily volume; the round targets are partly binning artefacts.
- If you are under about 4,000 steps a day you sit below every reported inflection point, on the steep part of the curve. Benefit appeared above about 3,143 steps a day in one meta-analysis and about 3,867 in another.
- Expect the bend between about 5,000 and 7,000 steps, then slow further improvement rather than a wall.
- Do not pay extra for cadence or intensity scoring yet. Three cohorts found no independent effect of intensity after adjusting for volume; two found one.

The most useful thing about this evidence base is not the number it produces but the number it dismantles. For a sedentary person a target of 10,000 is both arbitrary and demoralising, far enough away that missing it reads as failure rather than progress. The curves say something better and much less marketable: the steepest returns in the whole relationship belong to the people currently moving least, and they show up somewhere around three or four thousand steps a day. Whether walking causes that benefit or merely marks the people who were going to do well anyway is a question these cohorts cannot answer. What they can tell you is that the interesting part of the graph is nowhere near the number printed on the box.

## The takeaway

The mortality curve for daily steps bends between roughly 5,000 and 7,000 a day, and the steepest gains belong to people currently walking least, around three to four thousand. 10,000 was a pedometer's product name, not a threshold.
