Serum ferritin is the standard window into the body's iron stores — and iron status bends both ways: deficiency drags down energy and resilience, while overload tracks with diabetes and fatty liver. Between those poles, the number predicts far less than people hope.

There is a particular kind of exhaustion that sends people searching for a number. Not the tiredness of a bad week, but the flat, durable kind — the kind that makes a person wonder whether something measurable is wrong, whether a vial of blood might contain an explanation and therefore a fix. Of all the numbers that search turns up, ferritin may be the most emotionally loaded: the body's iron account balance, printed to one line on a lab report.
The stake is real on both ends. Iron is essential — for the hemoglobin that carries oxygen, for cellular energy — and toxic in excess. Ferritin, the main intracellular iron-storage protein, leaks into serum in proportion to the stores, which is why its measurement became the standard, widely used laboratory assessment of iron status. The hope in the vial is that one number will say where you stand.
The precise question is what that number can honestly tell you. The defensible answer: it is an early and specific indicator of deficiency and a marker of overload — and a poor fortune-teller for everything in between.
The strongest evidence sits at the bottom of the range. A low ferritin is an early and specific signal of iron deficiency — the body's leading cause of anemia — and the deficiency can hurt before anemia ever appears: iron deficiency without anemia is associated with fatigue that iron repletion can improve, a point developed in a 2015 review in the American Society of Hematology's education program. In older adults, deficiency has been linked to more severe infections. Pediatric literature, including a 2017 review in Archives de Pédiatrie on the biological diagnosis of iron deficiency in children, treats ferritin as a cornerstone. A 2021 Cochrane review assessed serum ferritin as an index of both deficiency and overload — the measurement itself, graded.
The other pole has its own dossier. Elevated ferritin and iron overload track with insulin resistance, type 2 diabetes, metabolic syndrome, and fatty liver disease — an entity now formalized as metabolic hyperferritinemia, examined in a 2024 record in the Journal of the Endocrine Society — and, in some populations, with higher mortality. A 2005 study in the Journal of the American Society of Nephrology mapped time-dependent associations between iron and mortality in hemodialysis patients. Iron status, in other words, has a U-shaped relationship with health: trouble at both ends.
Now the complications, which are considerable. Ferritin is also an acute-phase reactant: inflammation, infection, liver disease, and some cancers all raise it, which can mask a true deficiency and counterfeit an overload — one reason it is so often read alongside other iron markers such as transferrin saturation rather than trusted alone. Much of the alarming evidence at the high end is observational and confounded by exactly that inflammation. And within the normal range, ferritin was not associated with all-cause, cardiovascular, or cancer mortality in the large NHANES III cohort — a nationally representative American population, followed for outcomes that matter. The middle of the curve, where most lab reports land, is where the number says least.
For a reader, the practical meaning is that ferritin is a diagnostic instrument, not a horoscope: powerful for confirming deficiency or overload, interpretable only in clinical context, and easily distorted by a week of inflammation.
The exhausted person does get a number from the vial. But the number, it turns out, needs a translator — and the translation is most reliable precisely at the extremes where the exhaustion was pointing all along.
