The Omega-3 Index promises to turn fish-oil status into a single percentage you can track. The biomarker is scientifically serious — and the leap from a higher index to a longer life remains unproven.

So here's the thing about the Omega-3 Index: it's a number — one single percentage, arrived at from a finger-prick dried-blood spot — that claims to tell you how much of two particular fatty acids (EPA and DHA, the long-chain marine ones, the ones in fish oil that everybody has an opinion about) is built into the membranes of your red blood cells, and because those cells turn over slowly, the number reflects your long-term status rather than last Tuesday's salmon, which is either reassuring or faintly judgmental, depending on your relationship with last Tuesday.
And the pitch practically writes itself: test, learn your percentage, nudge it toward the commonly cited target zone of 8 to 11 percent, feel quantified. But the question that matters is narrower and harder: does this number actually predict anything, and — this is the part where the omega-3 conversation usually gets shifty — does raising it cause better outcomes, or does it just travel with them?
The index was proposed by Harris and von Schacky in a 2004 Preventive Medicine paper that asked, in its title, whether the Omega-3 Index was a new risk factor for death from coronary heart disease. Twenty-plus years later, the observational answer is about as consistent as nutritional epidemiology gets. A pooled analysis of blood n-3 fatty acid levels across 17 prospective studies, published in Nature Communications in 2021, linked higher levels to lower total and cause-specific mortality. In the Framingham Heart Study, reported in the Journal of Clinical Lipidology in 2018, erythrocyte long-chain omega-3 levels were inversely associated with both mortality and incident cardiovascular disease. A 2016 study of patients referred for coronary angiography found the same direction of association. Across ten cohorts, an index near 8 percent carried roughly 30 percent lower fatal coronary heart disease risk than one near 4 percent. A low index meets proposed criteria for a novel cardiovascular risk marker — that's a formal designation, not marketing copy. Lower erythrocyte omega-3 levels have also been linked to smaller brain volumes and poorer cognitive measures during aging.
Several large randomized supplementation trials produced neutral results for clinical endpoints. Read that again, because it's the hinge of the whole subject. The biomarker tracks outcomes beautifully in cohorts, and yet giving people omega-3 supplements in trials mostly did not move the hard endpoints. The field attributes the discrepancy partly to trial-design and bioavailability issues — doses too low, durations too short, populations already replete — rather than to a failure of the biomarker itself. That defense may be right. It is also unfalsified. And in 2024, a UK Biobank analysis tied regular fish-oil use in people without cardiovascular disease to higher risks of atrial fibrillation and stroke — observational again, but a signal pointing the opposite way from the marketing.
Everything strong in this literature is association. People with an index near 8 percent differ from people near 4 percent in dozens of ways — diet, income, exercise, smoking — that no adjustment fully erases. The 30-percent figure is a relative difference in observational data, not a promise that a supplement and a follow-up test will cut your risk by a third. What the index can honestly do is measure your long-term EPA plus DHA status with a standardized method; what it cannot yet do is certify that changing the number changes your fate.
If you're the quantified-self type, the Omega-3 Index is one of the more defensible panels you can order: it measures something real, tracks with outcomes across dozens of cohorts, and reflects diet over months rather than days. Just hold the number with the right-sized grip — as information about status, not as a scoreboard for immortality.
One finger-prick, one percentage, one small blood spot drying on a card. It is a genuinely clever measurement of a genuinely uncertain promise — which, in the supplement universe, puts it well ahead of most.
