What it is

Diclofenac is a non-steroidal anti-inflammatory drug (NSAID) that blocks the cyclo-oxygenase enzymes COX-1 and COX-2 — the catalysts that convert arachidonic acid into prostaglandins, the signalling molecules that sensitise nerve endings and drive inflammatory pain and swelling. Rubbed as a 1% gel directly over a painful joint, it builds high drug concentrations in the local skin, muscle and joint tissue while keeping blood levels a small fraction of those from a diclofenac pill — which is why it carries far less gastrointestinal and cardiovascular risk than oral NSAIDs.

Its role in longevity

This is the one over-the-counter pain topical with genuinely strong evidence. A Cochrane review of 39 trials (10,631 people) found topical diclofenac significantly outperformed its carrier in osteoarthritis, with roughly 60% of users reporting much-reduced pain, and a 2025 meta-analysis of 12 RCTs confirmed gel, solution and patch forms all improve pain and function in knee osteoarthritis with low side-effect rates. Best supported for hand and knee osteoarthritis.
