Phosphatidylcholine (PC) is a phospholipid that forms a major structural component of cell membranes and is the principal dietary form of choline. It is synthesized mainly in the liver (through the CDP-choline pathway and by PEMT-mediated methylation of phosphatidylethanolamine) and is obtained from foods such as eggs, soybeans, liver and meat, with supplements typically derived from soy or sunflower lecithin. Purified pharmaceutical preparations called essential phospholipids, or polyenylphosphatidylcholine (PPC), are enriched in 1,2-dilinoleoylphosphatidylcholine and have been studied as hepatoprotective agents, while delayed-release PC has been tested as a topical mucosal therapy in ulcerative colitis. Phosphatidylcholine also supplies choline for the neurotransmitter acetylcholine and is required for hepatic lipid export. Oral PC supplements should not be confused with unapproved injectable "fat-dissolving" phosphatidylcholine products, which have drawn FDA safety warnings.
In human health, phosphatidylcholine's most encouraging evidence is in ulcerative colitis, where several randomized trials and a meta-analysis of delayed-release formulations reported higher remission and endoscopic-improvement rates than placebo — but the pivotal modern program (LT-02) failed both of its large phase 3 trials in 2024, leaving the therapeutic question unresolved. As essential phospholipids it is widely used for liver disorders and improves some biochemical and steatosis measures in smaller studies, but the large Veterans Affairs randomized trial found no effect on progression of alcoholic liver fibrosis, a NAFLD nutraceutical-mixture trial showed no efficacy, and a Cochrane review of 12 trials found no clear benefit of lecithin for dementia or cognitive impairment. A 2025 cohort-plus-trial study linking lecithin intake to protection against combined sarcopenia and cognitive decline (via a proposed muscle-brain signaling pathway) is intriguing but preliminary and unreplicated. A notable safety caveat is that gut microbiota convert dietary phosphatidylcholine to trimethylamine-N-oxide (TMAO), which large human cohorts associate with higher cardiovascular and all-cause mortality risk. This body of evidence characterizes the phosphatidylcholine compound and its mechanisms rather than this specific commercial product, which has not itself been evaluated in these studies.
