Five Milligrams of an Element, and the Strange Evidence Trail Behind It

Low-dose lithium orotate sits in a peculiar evidence gap: prescription lithium prevents suicide in mood disorders, trace lithium in drinking water correlates with less dementia, and a landmark 2025 study implicated brain lithium deficiency in Alzheimer's — but no randomized trial has ever tested 5 mg orotate for cognition, mood, or aging.

Begin with the inventory of what a bottle of low-dose lithium orotate actually contains: a salt of the trace element lithium and orotic acid, delivering a few milligrams of elemental lithium per serving. Now the inventory of what psychiatry prescribes: lithium carbonate, roughly 150 to 1,200 milligrams a day, a mood stabilizer with real toxicity at high doses. Then the inventory of where the element turns up uninvited: drinking water, grains, vegetables, in variable trace amounts. Finally, the inventory of what the cell does with it: inhibition of glycogen synthase kinase-3β and inositol monophosphatase, actions linked to increased neurotrophic signaling, autophagy, and reduced phosphorylation of tau protein. Four lists, one element, and a gap between them wide enough to park a supplement industry in.

The question is whether the microdose bottle inherits any of the evidence gathered at the other doses. The most defensible answer: it inherits a mechanism and a suggestion — and no completed randomized trial of its own.

The one thing lithium has proven

The strongest human evidence in this entire file belongs to prescription lithium. A 2013 updated systematic review and meta-analysis in the BMJ found that lithium reduced suicide and all-cause mortality in people with mood disorders. That is a mortality benefit from randomized evidence — the rarest and most valuable kind — and it belongs to doses tens to hundreds of times higher than anything in a supplement capsule. It proves the element can matter profoundly to the brain. It does not prove that five milligrams does anything.

The cognitive trail

The Alzheimer's thread is newer and stranger. A 2015 systematic review and meta-analysis in the Journal of Alzheimer's Disease examined lithium as a treatment for the disease, and small trials with their meta-analyses suggest subtherapeutic or microdose lithium may slow cognitive decline in mild cognitive impairment and Alzheimer's. A 2024 network meta-analysis in Ageing Research Reviews placed lithium alongside the anti-amyloid antibodies donanemab, lecanemab, and aducanumab on cognitive outcomes in MCI and Alzheimer's. Then came the landmark 2025 Nature study: brain lithium was found reduced in mild cognitive impairment, and lithium orotate reversed Alzheimer's-like pathology — but the interventional arm was in mice, not a clinical trial, and the accompanying caution from experts was explicit: do not self-treat before human trials are done. Imaging studies do confirm one concrete fact: supplement-level doses measurably reach the human brain. The molecule gets there. What it then does in humans, at those doses, over years, remains untested in any randomized trial.

The ecological mirage

The third leg of the case is observational. A 2011 study in the European Journal of Nutrition linked low-dose lithium uptake to longevity in humans and metazoans; ecological and cohort studies connect higher natural lithium in drinking water to lower rates of dementia and suicide; low-dose lithium extends lifespan in model organisms. But these findings are inconsistent — some null, some nonlinear, all potentially confounded by everything else that distinguishes one water supply, one town, one population from another. Ecology can point; it cannot prove. Whether the orotate salt is meaningfully different from other lithium forms remains debated.

For a reader, the practical meaning is caution shaped like an inventory: proven benefit at psychiatric doses, intriguing signals below them, animal reversal in a landmark paper, and not one randomized trial of 5 mg lithium orotate for cognition, mood, or aging. The lists are real. The bridge between them has not been built. Until human trials cross it, the bottle of five milligrams remains what the evidence says it is: a trace of an element with a remarkable résumé, taken on speculation.

The defensible position: lithium is a biologically active element with striking observational and animal signals at low doses, one proven benefit at prescription doses in psychiatric illness, and zero completed randomized trials of supplement-level lithium orotate for cognition, mood, or aging.
