Topical Pain Relief: What the Evidence Actually Supports

Creams, gels and patches all promise fast relief — here is which mechanisms hold up in human trials, ranked by the evidence.

Pain is private. The wish behind rubbing something onto a sore joint is not about molecules — it is to treat the exact spot that hurts and leave the rest of the body out of it. It is also plausible: a topical builds a high drug concentration where it hurts while keeping blood levels low — fewer stomach, kidney, and heart risks than swallowing the same drug.

But “topical” covers everything from rigorously tested anti-inflammatories to cooling gels that mostly feel nice. Sorted by human evidence, the options run from topical diclofenac — the standout for reachable arthritis — through capsaicin and lidocaine, down to menthol and medicated patches: comfort, honestly labeled.

Strong evidence: topical diclofenac (an NSAID)

Topical diclofenac — the active ingredient in Voltaren Arthritis Gel — is the standout. It blocks the COX enzymes that manufacture inflammatory prostaglandins, and it does so in the local tissue rather than your whole body. A Cochrane review pooling 39 trials and over 10,000 people found it clearly beat its inactive carrier for osteoarthritis, with roughly 60% of users reporting much-reduced pain, and a 2025 meta-analysis of 12 randomized trials confirmed gels, solutions and patches all improve both pain and function in knee osteoarthritis, with low side-effect rates. If you have hand or knee arthritis, this is the evidence-based first stop.

Moderate evidence: capsaicin and lidocaine

Capsaicin — the heat compound from chili peppers — works by overstimulating the TRPV1 receptor on pain nerves until they "defunctionalise" and quieten. The strongest data is for the high-concentration 8% prescription patch in nerve pain; for the over-the-counter 0.025–0.1% creams in knee osteoarthritis, a careful n-of-1 trial series found relief comparable to topical ibuprofen, with many people responding better to one than the other. It only works if you use it consistently for a week or two, and the initial burning is part of the deal.

Lidocaine numbs pain a different way — it blocks the sodium channels nerves use to fire. Its best evidence is the 5% medicated patch for postherpetic neuralgia (shingles nerve pain), where it is a guideline-recommended first-line option with minimal systemic absorption. A Cochrane review noted that large high-quality trials are thin even though individual studies and clinical experience support it, so treat the over-the-counter 4% creams and patches as a reasonable option for small, well-defined painful spots rather than widespread aches.

Emerging and comfort-level: menthol and medicated patches

Menthol gels such as Biofreeze trigger the TRPM8 cold receptor, producing a cooling sensation that competes with pain signals. Trials show it can speed up relief and add lasting cooling when combined with an NSAID — but a sprain study found a menthol/diclofenac combination no better than diclofenac alone, so menthol's real value is immediate comfort rather than deep healing. Methyl-salicylate-plus-menthol patches (the classic Salonpas) add a mild aspirin-like salicylate and the convenience of a hands-free patch; reviews list them among effective, well-tolerated options for everyday musculoskeletal aches, though their dedicated trial base is smaller.

How to choose

For arthritis in a joint you can reach, topical diclofenac has the science behind it. For nerve-type or pinpoint pain, consider lidocaine; for those who tolerate the burn and stay consistent, capsaicin. Menthol gels and medicated patches are low-risk ways to get fast, drug-light comfort. Whatever you choose, follow the label, don't stack a topical NSAID with oral NSAIDs without medical advice, and see a clinician for pain that is severe, spreading, or not improving.
