The Number on the Cuff: What Beetroot Juice Can and Cannot Do About It

Behind every beetroot shot is a person watching a blood-pressure reading edge upward with age. The nitrate science is real — about 5 mmHg on average — and the limits are real too.

There is a particular dread attached to the home blood-pressure cuff — the quiet ritual of the arm bared, the band tightened, the number arriving a few points higher than last year. It is one of the few places where aging announces itself as a single digit you cannot argue with. Millions of people watch that number climb and wonder whether something simple, something from the produce aisle, might push back. This is the emotional engine behind beetroot juice, and it deserves a sober answer rather than a sales pitch.

The question is precise: does the inorganic nitrate in beetroot — converted by bacteria on the tongue to nitrite, and then in the body to nitric oxide, the signaling molecule that relaxes blood vessels — lower blood pressure and improve physical capacity in measurable human trials? The most defensible answer is yes, modestly, on average, with important caveats about who responds and what has never been tested.

The blood-pressure evidence is the strongest layer

Meta-analyses of randomized trials show that beetroot juice and dietary nitrate lower systolic blood pressure by about 5 mmHg on average across trials in hypertension. In one pivotal placebo-controlled trial in hypertensive patients, the effect was roughly 7 to 8 mmHg — including on 24-hour ambulatory readings, not just office measurements — sustained over four weeks, alongside improved endothelial function and reduced arterial stiffness. For context, effects of that size at the population level are meaningful; for an individual, 5 mmHg is a nudge, not a rescue. Diastolic effects are inconsistent, and individual responses vary widely.

Exercise capacity: real, bounded, and dose-dependent

An umbrella review of 20 meta-analyses found improved exercise time-to-exhaustion, muscular endurance, and peak power, particularly at nitrate doses of 6 mmol or more per day. Systematic reviews in Nutrition Reviews and BMJ Open Respiratory Research reached broadly similar conclusions about exercise capacity and cardiovascular parameters. The performance literature is not all mechanism: a 2017 trial found beetroot juice outperformed sodium nitrate for attenuating muscle pain after strenuous eccentric exercise, suggesting the whole food may carry something beyond nitrate alone, and a 2025 study tested acute beetroot juice before climbing, measuring lower-body strength, aerobic power, and soreness in mountain climbers. Several studies suggest older adults may benefit specifically through improved vascular and muscle function — the very systems the home cuff is worried about.

Where the evidence goes quiet

The complication deserves prominence. Most trials are small and short. Trials in COPD and a 13-week cognition study found no benefit. No trial has tested the outcomes that matter most — stroke, heart attack, survival. A 2025 review of the nitrate–nitrite–NO pathway in peripheral artery disease describes therapeutic potential, which is the literature's careful way of saying the clinical proof is not in. And the whole mechanism has an achilles heel hiding in the bathroom cabinet: the effect depends on oral bacteria that convert nitrate to nitrite, and antiseptic mouthwash largely abolishes it. A pathway that mouthwash can silence is not a pharmaceutical-grade lever.

What this means in practice

Beetroot is one of the richest dietary sources of inorganic nitrate; spinach, rocket, and celery are also high in it, and betalain pigments may add antioxidant effects beyond nitrate. For a reader, the rational frame is food, not medicine: a plausible, low-cost contributor to vascular health with a measurable but modest average effect, best understood as one component of a blood-pressure strategy that a clinician should oversee — never a replacement for prescribed treatment.

The cuff will still inflate tomorrow, and the number will still be what it is. What the evidence offers is not a way to argue with it, but a small, honest lever: a vegetable, a bacterial conversion, a few points of pressure — real, limited, and worth exactly the hope it has earned.
