What Your Water Filter Takes Out, and What a Few Drops Can Honestly Put Back

An inventory of things involved in a glass of reverse-osmosis water. Item: a membrane that removes roughly 90 to 99 percent of the water's naturally occurring minerals. Item: what remains — very low-mineral, 'soft' water, remarkably pure and remarkably empty. Item: a small bottle of remineralization drops, adding back principally magnesium and calcium — the two ions that define water hardness, and the two that most drinking-water health research has focused on. Item: in the body, magnesium's resume — cofactor for hundreds of enzymatic reactions, participant (with calcium) in vascular tone, blood-pressure regulation, and the electrical activity of the heart. Item: one nationwide Dutch dataset on drinking-water exposure and mortality. Item: one 2018 study linking the softness of drinking water to higher lead content and raised cardiovascular morbidity. Item: one umbrella meta-analysis of magnesium and blood pressure. Item: one striking 2026 trial of mineral water in adults over fifty.

What does this odd collection add up to? The precise question is whether restoring minerals to demineralized water does anything measurable for health. The most defensible answer: the minerals themselves have real, modest evidence; the drops as a product have never been tested; and the water-hardness literature is genuinely split.

The strongest evidence belongs to magnesium

Randomized controlled trials, pooled in a 2024 umbrella meta-analysis, show that magnesium supplementation modestly lowers blood pressure — a 2016 review in Hypertension and related work establish the mineral's biological credentials. Observational cohort studies and meta-analyses link higher magnesium intake, including magnesium in drinking water, to lower blood pressure and reduced risk of stroke, heart failure, type-2 diabetes, and all-cause mortality. That is a broad and consistent pattern. It is also, in the drinking-water portion, almost entirely observational — associations flowing through pipes, not proof.

The closest direct trial — and its limits

No study has tested mineral drops themselves. The nearest thing is a 2026 double-blind randomized controlled trial in adults over 50, in which daily calcium- and magnesium-rich mineral water cut falls by 82 percent at six months and improved muscle mass over a year. That is a dramatic number, and it deserves two immediate qualifiers: it tested bottled mineral water, not drops, and it is one trial. Promising, specific, and not yet a literature.

The hardness question refuses to settle

Several cohort and case-control studies report fewer fatal heart attacks and strokes in areas with harder water. Others — a well-known analysis from north-west England, for example — find no association, and heterogeneity across studies is high. The Dutch nationwide assessment and the lead-and-softness study add texture rather than resolution: water chemistry interacts with pipes, plumbing, and populations in ways that resist clean conclusions. Meanwhile, a practical arithmetic problem: drops deliver small mineral doses relative to dietary needs. Food remains the main mineral supply line; water, even remineralized, is a tributary — even a diligent drop user drinking liters daily adds a fraction of what nuts, greens, or dairy provide.

What a reader should take from this

If you run reverse osmosis or drink distilled water, remineralization is a sensible restoration of what the membrane stripped — a fix for a man-made absence, not an upgrade to nature. Frame it honestly: a reasonable habit with plausible, modest upside, not a demonstrated longevity intervention. Anyone with kidney disease or on magnesium-affecting medication should discuss mineral intake with a clinician.

The glass on the counter is clearer than anything a river ever produced, and emptier too. A few drops return two ancient ions to it — magnesium, calcium, the chemistry of hardness — while the science, like the water itself, remains pleasingly, stubbornly unsettled.

Higher magnesium intake (including from drinking water) is observationally linked to lower blood pressure and cardiovascular risk, and magnesium supplements modestly lower blood pressure in trials — but no study has tested mineral drops themselves, and drops deliver small doses relative to dietary needs.
