The full story

In 2020 a research group reported that after 60 daily sessions of hyperbaric oxygen, the telomeres in older adults' immune cells were more than 20 percent longer and their senescent T cells had declined. It is an arresting result, and it is the reason a soft-shell chamber is now something people put in a spare bedroom. The distance between those two sentences is most of what this article is about.

A single finding can be true, important, and completely unable to bear the weight placed on it. This one is carrying a consumer category on its back, so it is worth looking at what the study actually was.

What the telomere study actually was

Published in Aging (Albany NY) in 2020: 35 healthy, independently living adults aged 64 and older. A prospective trial with no control group - every participant received the intervention, and there was nobody to compare them with. Sixty daily sessions. Telomere length rose by over 20 percent in T helper, T cytotoxic, natural killer and B cells, with the B-cell increase reported as 37.63 percent plus or minus 52.73. Senescent T helper cells fell 37.30 percent plus or minus 33.04, and senescent cytotoxic T cells fell 10.96 percent plus or minus 12.59.

Look at those plus-or-minus figures rather than the headline. In the B-cell result the standard deviation is larger than the mean change itself, which tells you participants did wildly different things and the average is concealing most of it. And every one of these measurements was made in isolated blood cells. Not in tissue, not in organ function, not in anything a person experiences. A cell-level readout in a single-arm study of 35 people, with dispersion that wide, is a hypothesis worth funding. It is not an outcome.

The skin follow-up, published in the same journal in 2021, has the same shape. Of a 70-person cohort, only 13 male participants consented to skin biopsies. After a three-month period of no intervention followed by three months of daily treatment, collagen density, elastic fibre length and blood-vessel number increased and tissue senescent cells decreased, with no change in elastic fibre density or thickness. Not randomised, not blinded, thirteen biopsies. That is a signal, and thirteen is thirteen.

A 2021 commentary in Rejuvenation Res, written about these very findings, describes the proposed mechanism - a hyperoxic-hypoxic paradox operating through HIF-1a - and then states plainly that the durability of the reported beneficial changes is yet to be determined and that the preliminary results require follow-up. That is the field's own assessment, published by people sympathetic to the hypothesis.

Where the trials get better, and what they were about

Hyperbaric oxygen is not fringe. A randomised controlled trial in Aging in 2020 gave 63 healthy adults over 64 either treatment or control over three months and found a significant group-by-time interaction for global cognitive function, with the largest effects on attention and information-processing speed and increased regional cerebral blood flow on perfusion MRI. Read the comparator carefully, though: it was a no-treatment control arm, not a sham chamber. Participants knew they were being treated, and daily attendance in a sealed chamber is about as high-ceremony as placebos come.

That matters, because a sham-controlled trial is clearly possible. A randomised, double-blind, sham-controlled trial in Sci Rep in 2022 ran 40 daily sessions in 73 patients with post-COVID condition and reported improvements in global cognition, attention, executive function, energy, sleep, psychiatric symptoms and pain interference, with corresponding brain MRI perfusion and microstructural changes. Whatever you make of the result, the design proves the point: nobody has to run an unblinded hyperbaric trial.

What the evidence does not show

For the same indication, an independent read came out the other way. A 2024 living systematic review in BMJ covering 24 randomised trials and 3,695 patients found no compelling evidence to support the effectiveness of hyperbaric oxygen for long covid, while moderate-certainty evidence supported online cognitive behavioural therapy and combined physical and mental rehabilitation. One positive sham-controlled trial; one systematic review concluding the case is not made. Both are in the record.

In children with cerebral palsy the verdict is harder. A 2022 systematic review in PLoS One of five randomised trials, four rated high level of evidence, all using 100 percent oxygen at 1.5 to 1.75 atmospheres absolute, reports high-level evidence that hyperbaric oxygen is ineffective for improving motor and cognitive function, plus moderate-level evidence that it produces more adverse events than pressurised air - with middle-ear barotrauma affecting up to half of the children treated. High-level evidence of no benefit is a far stronger statement than an absence of evidence.

Even at the established end, durability is a problem. A 2015 systematic review in Cochrane Database Syst Rev, 12 randomised trials and 577 participants, found adjunctive hyperbaric oxygen improved healing of diabetic foot ulcers at six weeks, but the benefit was not evident at one year, and there was no statistically significant reduction in major amputation. The review authors said design and reporting flaws left them not confident in the results - and this is the indication with the most credibility in the whole file.

One more absence worth stating outright: nothing in this literature is a study of a home chamber. The only pressures reported anywhere in these papers are the 1.5 to 1.75 atmospheres used in the cerebral palsy trials, which is the set of trials that came out negative. Whatever a soft-shell unit in a spare room delivers, no trial here tested it, and the 60-session protocol everyone cites was not run on one.

The arithmetic of sixty sessions

Anyone contemplating that protocol should look at the denominators. A retrospective analysis in Undersea Hyperb Med of 2,334 patients treated over 2010 to 2014 at a single hyperbaric centre found 406 patients, 17.4 percent, experienced at least one adverse event, at an overall per-session incidence of 0.72 percent. Middle-ear barotrauma was commonest, in 9.2 percent of patients, with higher risk in females and in children under 16. A 2015 series in Asia Pac J Clin Oncol of 189 patients treated for chronic radiation injury reported a 15.9 percent total complication rate, mostly reversible ear barotrauma at 10.6 percent and reversible ocular barotrauma at 4.2 percent. Note that the per-session numbers are small and the per-course numbers are not, which is the arithmetic a 60-session commitment turns on.

Then there is the regulatory picture, which is blunter than the wellness framing suggests. Hyperbaric chambers are Class II medical devices reviewed through the FDA's 510(k) pathway, listed under product code CBF and designated life-sustaining or life-supporting. In August 2025 the FDA wrote to health care providers stating that it was aware of recent reports of fires in hyperbaric oxygen devices that resulted in serious injuries and deaths, warning that there is a heightened risk of fire with use of oxygen at a high concentration, and advising that manufacturer instructions be followed, that electrical or static-generating devices be kept out of the chamber, that only hyperbaric-compatible clothing be used rather than synthetic fabrics or wool, and that staff be trained and patients monitored. Staff. Monitored. Those words appear in the guidance for a reason, and a spare bedroom does not have either.

The honest summary is that hyperbaric oxygen is a real medical intervention with real indications, real adverse events, and one uncontrolled study of blood cells that got mistaken for a rejuvenation protocol. If the telomere finding is right, it deserves a randomised, sham-controlled trial with tissue-level outcomes and long follow-up - the study the field's own commentators say is needed. That trial has not been run. Until it is, buying a chamber is not acting on evidence; it is pre-ordering a result.
