Mushrooms are the one food group where ergothioneine — an unusual dietary antioxidant that humans have a dedicated transporter for — shows up in meaningful amounts, and shiitake and oyster mushrooms sit near the top of the table. The case that it matters is intriguing but unfinished: one much-cited paper asked whether ergothioneine is a "longevity vitamin" missing from modern diets (Journal of Nutritional Science, 2020), and a Swedish cohort found that people with higher plasma ergothioneine went on to have lower cardiovascular disease and lower overall mortality (Heart, 2020). Both are observational; neither proves that eating more mushrooms changes outcomes.

The brain data carry the same asterisk. In a community study of older adults in Singapore, those eating more than two portions of mushrooms a week had markedly lower odds of mild cognitive impairment (Journal of Alzheimer's Disease, 2019) — a cross-sectional snapshot, not a trial. Mushrooms also contribute spermidine, a polyamine that extended lifespan and protected hearts in mice (Nature Medicine, 2016), with human evidence still catching up. The garlic and shallots in the dressing have their own literature: a meta-analysis found garlic preparations lower blood pressure in hypertensive people (Journal of Nutrition, 2016), though at supplement doses larger than a larb's.

Why this works

Shiitake and oyster mushrooms carry the ergothioneine, which is heat-stable, so a hard sear costs you flavor-wise nothing and compound-wise little; the same mushrooms bring the spermidine. Garlic and shallots supply allicin precursors — crushing the garlic and letting it rest ten minutes before it hits any heat gives the enzyme that forms allicin time to work. Toasted rice powder adds the nutty crunch that makes larb feel like larb, lime and fresh mint keep it bright, and butter lettuce turns the whole thing into a light, low-effort wrap.

How to make it

  1. Toast 2 tbsp raw sticky or jasmine rice in a dry pan until deep golden, then grind to a coarse powder.
  2. Finely chop 250 g shiitake and 250 g oyster mushrooms. Crush 2 garlic cloves and set them aside to rest.
  3. Sear the mushrooms in a barely oiled, very hot pan without stirring much, 6–8 minutes, until browned and dry at the edges.
  4. Off the heat, toss with the garlic, 2 thinly sliced shallots, juice of 2 limes, 1½ tbsp fish sauce (or tamari) and a pinch of chile flakes.
  5. Fold in a big handful of torn mint and cilantro plus most of the rice powder.
  6. Spoon into butter lettuce cups and finish with the remaining rice powder and lime wedges.

Serves 3–4 as a light main. The filling is best warm, assembled at the table so the lettuce stays crisp.

The takeaway. The ergothioneine and mushroom-cognition findings are observational — encouraging enough to make mushrooms a regular habit, not proof they protect your heart or memory. This larb just makes the habit easy to keep.
