Is serum uric acid (SUA) level associated with all-cause and cardiovascular disease (CVD) mortality in individuals with diabetes?
Diabetes Care · 2023 · PMID 36490263 · DOI 10.2337/dc22-1339
Higher serum uric acid levels are linked to increased mortality risk in people with diabetes.
The question
Is serum uric acid (SUA) level associated with all-cause and cardiovascular disease (CVD) mortality in individuals with diabetes?
What they tested
The study investigates an association between serum uric acid levels and mortality, implying a potential link.
How they did it
This prospective cohort study analyzed data from 7,101 patients with diabetes from the U.S. National Health and Nutritional Examination Survey (NHANES) 1999-2018. Mortality data were linked to national death records through December 31, 2019. Weighted Cox proportional hazards regression models were used to calculate hazard ratios (HRs) for all-cause and CVD mortality. A meta-analysis of 13 additional cohort studies was also performed.
What they found
Among NHANES participants, the weighted mean SUA level was 5.7 mg/dL. During 57,926 person-years of follow-up, there were 1,900 deaths, with 674 attributed to CVD. Patients in the highest SUA quintile had an HR of 1.28 for all-cause mortality and 1.41 for CVD mortality compared to those in the lowest SUA quintile. The meta-analysis of 13 cohort studies showed pooled HRs of 1.08 for all-cause mortality and 1.05 for CVD mortality per 1 mg/dL increment of SUA.
| All-cause mortality (highest vs. lowest quintile) | 1.28 | |
|---|---|---|
| CVD mortality (highest vs. lowest quintile) | 1.41 |
Hazard ratios for all-cause and CVD mortality when comparing the highest serum uric acid quintile to the lowest quintile in diabetes patients.
What it means
Higher serum uric acid levels are associated with increased risks of both all-cause and cardiovascular disease mortality in individuals with diabetes.
Limitations
The study design is observational, which means it can identify associations but not prove causation. The abstract does not explicitly state other limitations.
Where the published abstract does not list limitations, we say so rather than inventing them. Read the full paper on PubMed before drawing conclusions.
